Document Upload Question (Cobalt 3.2+)

Document Upload questions allow customers to upload documentation without custom development. Following the steps in this article you should be able to add these questions to your forms and map them so that you can locate files, which will be attached as Notes when the form and related process are submitted.

1. Set Question Type as Document Upload

2. Set the Name to what you are asking for (Copy of a License, Signed Letter, Image, etc)

3. Determine other settings (Hidden, Required etc) as needed

4. Create a Question Mapping with the Type To

5. Launch Mapping Wizard in the toolbar

Select the type of Form you are starting From. The example above is if you want to Map to the Meeting Registration Entity.

Start From the Meeting Registration (Form Response)

Mapped Field =Note

Note Subject = You can give this any name as this is what will appear as the title of your note when you go to look for it on the Meeting Registration Record.  

The example above shows how it will look if you go from the starting entity to a different entity.

For Certification Applications, Membership Applications, and Class Registration you may not be able to add the note to the application/registration so you can link it to the Contact Record.

Start From the (Continuing Certification Application, Initial Certification Application, Membership Application, Class Registration)

Map to Related Entity = Contact

Mapped Field = Note

Note Subject =You can give this any name as this is what will appear as the title of your note when you go to look for it on the Contact Record

 

The example above shows the mapping for Meeting Companion Registrations. These cannot link to a contact or to the companion registration, but they can be linked to the Meeting Registration for the person adding the companion.

Start From the Meeting Companion Registration

Related Entity = Meeting Registration

Mapped Field = Note

Note Subject = You can give this any name as this is what will appear as the title of your note when you go to look for it on the Meeting Registration Record.  

6. Save Mapping

7. Add Form to your application/registration record in CRM (if not done so already) and test on the portal and/or wizard process. Your question should appear similar to the image below
