Form: Entity Definition

ADMIN --- FORMS

 

Cobalt's Forms Functionality allows you to collect information from your end users as part of a meeting or class registration and membership application processes. Each Form has Pages, Sections, and Questions.

INFO

  • Name - The name of the form.
  • Description - A description of the form used for internal staff use.

Page - Current pages will be available to view, select + to add new pages to the form

  • Name - The name of the page (our example has it the same because there is one page, but they do not have to be the same as the form)
  • Sort Order - If the form has multiple pages, this field determines the order in which they will be displayed.

Section - Current sections will be available to view, select + to add new sections to the page

General Tab

  • Name - The name of the section.
  • Page - The page containing this section.
  • Sort Order - If the page has multiple sections, this field determines the order in which they will be displayed.

Questions - Current questions will be available to view, select + to add new questions to the section.

QUESTION

  • Name - The name of the question.
  • Type - The type of answer you want for the question
    •  Single Line of Text - Allows respondent to type in information to short answer questions 
    • Option Set (picklist) - Allows respondent to select from options (Options must be created in Answers record for this type).
    • Two Options (bit) - Allows respondents to select from two options, often used for yes/no questions (Options must be created in Answers record for this type).
    • Whole Number (int) - Allows respondents to answer with a whole number value
    • Floating Point Number (float) - Allows respondents to have an answer that may have a decimal somewhere
    • Decimal Number (decimal) - Allows respondents to answer with a number that includes decimal points.
    • Currency (money) - Allows respondent to answer with a monetary value.
    • Multiple Lines of Text (ntext) - Allows respondent to type an answer that may require a detailed answer (nothing needed in Answers record for this type).
    • Date (datetime - Date only) - Allows respondent to give a date.
  • Sort Order - If the section has multiple questions, this field determines the order in which they will be displayed.
  • Section - The section associated with the question.

VALIDATION

  • Required - If you require a response select Yes. If the question is optional select No.
  • Required Validation - If you require a response you must include validation
  • Format (regex) - 
  • Format Validation - 

ANSWERS - Current answers will be available to view, select + to add new answers to the question.

  • Display Value - What the respondent sees when they answer the question
  • Value - System value may be different than the display but does not have to be
  • Question - lookup field to a question associated with this answer
  • Sort Order - If there is more than one answer, the order this answer will appear in the options.
