• Name - An Automatically populated field recording the name of the certification and the applicant.
• Application Type - A lookup to the Certification Type for which the contact is applying.
• Contact - The contact applying for continuing certification.
• Form Response - A lookup to responses provided to any forms you created for the application.
• Certification Fee - A lookup to the fee charged to apply for this certification.
• Certification - The certification currently held by the contact.
• Date Submitted - The date the application was submitted.
• Flagged for Audit - Field that will let you know if this application has been flagged for auditing.
